Immunization

Procedures for Proof of Immunization

Full or part-time Gateway students enrolled in a degree or certificate program or if you are a Non-Degree student registering for 12 or more credits who were born after December 31, 1956, must present proof of adequate immunization against both measles and rubella. Proof includes such documentation as a medical record, a physician's statement or an elementary or secondary school record. Click here to download an Immunization Form.

Note: Any Connecticut high school graduate, 1999 and beyond, is exempt from providing traditional documentation of MMR compliance. However, proof of high school graduation in the form of a diploma or transcript must be on file in order for immunization to be waived.
